Alaska Native Tribal Health Consortium has a hiring preference for qualified Alaska Native and American Indian applicants pursuant to P.L. 93-638 Indian Self Determination Act. Summary Under limited supervision, performs clinical pharmaceutical care for patients in collaboration with other pharmacists, physicians, other healthcare providers and patients in an accurate and timely manner. Responsibilities All Provides patient counseling, drug information, drug therapy consultation, and staff education. Reviews patient profile and clinical status, labs, dosing intervals, and drug therapy combinations to assure safe and effective medication use. Ensures maintenance of an accurate medication list for all patients. Consults with healthcare providers on therapy recommendations as appropriate. Educates patients regarding the proper use of medication and what side effects to expect. Conducts medication dispensing, compounding, order entry, and verification of all pharmaceutical activities. Repackages and distributes prescriptions; performs compounding of IV, topical, and oral medications in an accurate and timely manner. Documents patient and procedural information. Accountable for adhering to all pharmacy billing practices, formulary restrictions, and related policies and procedures to maximize revenue opportunities while ensuring patient safety standards are met. Reviews and authorizes pharmacy technician activities. Responsible for any delegated act performed by pharmacy technicians. Participates on various committees. Ensures compliance with the regulatory and practice standards of pharmaceutical care. Performs Other Duties As Assigned Or Required. Pharmacist II Involvement in one committee. Pharmacist III Involvement in 2+ committees OR clinical specialist in respective area. Pharmacist IV In addition to duties of Pharmacist II and III listed above, serves as a source of leadership across departments AND serves as a subject matter expert in at least one area or initiative. Other Information KNOWLEDGE and SKILLSA Doctorate degree in Pharmacy (BS for candidates who were licensed under the BS License requirement) from an accredited college program.
